To write in the ‘Actions’ sections for a care plan you first have to complete a risk assessment.

When you fill out the ‘Actions to mitigate this risk’ box within a risk assessment, this will show up in the ‘Actions’ section in the care plan. The ‘Actions to mitigate this risk’ text box is found at the very bottom of a risk assessment.

To edit the Actions box you have to go into the relevant risk assessment and edit the ‘Actions to mitigate this risk’ box.
