How do I add a risk assessments to a care plan?

Risk assessments can be added to care plans to determine who is at risk, evaluate the risks and implement safety measures.

You can add a specific risk assessment to none, one or multiple care plans. There are three ways you can add a risk assessment to a care plan:


Option A) Add a risk assessment directly from the Risk Assessments Dashboard

Step 1 – Go to the 'Risk Assessments' tab in the client profile

Step 2 – Click 'New risk assessment'

A new risk assessment can be added from the risk assessment tab by selecting the 'new risk assessment' button.

Step 3 – Choose the risk assessment you would like to add

You'll see a list of risk assessment templates to choose from. You can do this by either looking through the list or using the search bar.

If you choose a risk assessment that has already been completed, you'll get the choice to attach the existing risk assessment to a care plan or fill out a blank assessment.

If you want to attach the existing assessment to a new care plan, please follow the next steps described here.

Step 4 – Attach to care plans

Open up the drop-down 'Attached to care plans' and select the care plans you want this risk assessment to be linked to. You can only select care plans that have been filled in and thus been reviewed at least once.

Step 5 – Fill out the risk assessment

If the risk assessment has an ‘Actions to mitigate this risk’ text box, this will automatically show up under the ‘Actions’ section in the care plan.

Step 6 – Press 'Save'

Once you click on the Save button, a pop-up will appear reminding you that you'll need to review the care plans you've attached the assessment to. Click ok on this.

You’ll now see that the risk assessment has been added to the ‘Risks’ section in the care plan and also in the risk assessment dashboard.


Option B) Add a risk assessment directly from the care plan

Step 1 – Go to the care plan that you want to add the risk assessment to

Step 2 – Press ‘Review Care Plan’

If you want to change anything on a care plan, you always have to first press ‘Review Care Plan’ first.

Step 3 – Click ‘Add Risk Assessment’

Step 4 – Choose the risk assessment you would like to add 

You can do this by either looking through the list or using the search bar.

If you choose a risk assessment that has already been completed, you'll get the choice to attach the existing risk assessment to a care plan or fill out a blank assessment.

If you decide to attach the completed assessment, you are done after clicking the button. Otherwise continue with Step 5.

Step 5 – Choose if you would like to fill it out now or later

Now – If you've chosen now you can go ahead to the next step and fill out the risk assessment.

Later – If you choose to fill it out later we’ll add a blank risk assessment to the care plan for it to be filled out at a later date. It will also say ‘Incomplete’ underneath the title. 

Step 6 – Attach to care plans

Open up the drop-down 'Attached to care plans' and select the care plans you want this risk assessment to be linked to. You can only select care plans that have been filled in and thus been reviewed at least once.

Step 7 – Fill out the risk assessment

If the risk assessment has an ‘Actions to mitigate this risk’ text box, this will automatically show up under the ‘Actions’ section in the care plan.

Step 8 – Press ‘Save’

You’ll now see that the risk assessment has been added to the ‘Risks’ section in the care plan and also in the risk assessment dashboard.


Option C) Attach an existing risk assessment to care plans

Step 1 – Open the risk assessment and click edit

You can do this by clicking on the risk assessment from the Risk Assessments tab or from a care plan that it's already attached to.

Step 2 – Attach to care plans

Open up the drop-down 'Attached to care plans' and select the care plans you want this risk assessment to be linked to. You can only select care plans that have been filled in and thus been reviewed at least once.

Step 3 - Press save

Once you click on the Save button, a pop-up will appear reminding you that you'll need to review the care plans you've attached the assessment to. Click ok on this.

You’ll now see that the risk assessment has been added to the ‘Risks’ section in the care plan and also in the risk assessment dashboard.
