There are three ways to update an existing risk assessment:
A. Update risk assessment from the 'Risk Assessments' tab
Step 1: Go to 'Risk Assessments' and select the risk assessment
Open the Risk Assessment tab in your client's profile. Here you'll see an overview of all risk assessments you've completed for the client. Click on the title of the one you want to edit.
Step 2: Click edit
Step 3: Update Risk Assessment
Step 4: Click 'Save'
The last reviewed date will now change to the current date and time. The risk assessment will update in all care plans that it's attached to and you'll be prompted to review them.
You can always view any of the past revisions of a risk assessment if needed.
B. Update risk assessment from the care plan - view mode
Step 1: Open care plan and select the Risk Assessment you want to edit
Open the care plan that has the relevant risk assessment attached. You don't have to click edit yet.
Step 2: Click edit
Step 3: Update Risk Assessment and click 'Save'
Now you can make changes to the risk assessment.
The last reviewed date will now change to the current date and time. The risk assessment will update in all care plans that it's attached to and you'll be prompted to review them.
Click 'Back' to leave the care plan.
You can always view any of the past revisions of a risk assessment if needed.
C. Update Risk Assessment from the care plan - edit mode
If you're already updating a care plan, you can also update the risk assessment at the same time.
Step 1: Open care plan and click 'Review Care Plan'
Open the care plan that has the relevant risk assessment attached and click 'Review Care Plan'
Step 2: Select the Risk Assessment you want to edit
Scroll to the risk assessment you want to edit and click on its title.
Step 3: Update Risk Assessment
Now you can make changes to the risk assessment.
Step 4: Click 'Save'
Continue editing the care plan and save it once you're done.
The last reviewed date will now change to the current date and time. The risk assessment will update in all care plans that it's attached to and you'll be prompted to review them.
You can always view any of the past revisions of a risk assessment if needed.
