Monitor your client's Risk Assessment scores over time

The Risk Assessment report has been built to give managers an overview of all of their client's Risk Assessments, in one place. The chart keeps the care team informed of changes in risk assessment scores which they can monitor over time.

What are the benefits of this feature?

  • Informed decision-making: Empowers managers and the care team to make better-informed decisions. You can identify trends, patterns, and areas of concern much faster and adjust the care plans and interventions accordingly to mitigate risks.

  • Enhanced safety and care planning: By considering historical data, managers can develop strategies that address not only current risks but also anticipate potential future challenges.

  • Improved communication and coordination: Centralised risk assessment scores facilitate seamless team communication, providing a common reference point, and promoting an aligned approach to care delivery.

Where can I find the report?

To locate this report, head to the Reports tab, then on the left-hand side of the page, locate the Care Planning section which shows the Risk Assessments Scores.

Then, click on the drop-down menu to select a Risk Assessment.

Once you've selected a Risk Assessment, you can see individual client Risk Assessment scores, both current and past, with direct links to historical records for deeper analysis.

When viewing the chart in the Care Office, you can also filter by time, Risk Assessment title and client name.
