On the risk assessment dashboard, you can see a list of all the risk assessments you’ve written for this client.
To add, remove or update any risk assessment you can either go into the care plan that it is attached to or make any alterations directly from the risk assessment dashboard.

In the dashboard you can also view which care plan a risk assessment is attached to. Simply hover over the blue link saying "Contains X Risk Assessments" and you'll be able to see which care plans the risk assessment is linked to.
If you click on one of the care plans that pop up, you'll be taken to that care plan directly.
Risk assessments that aren't attached to any care plans can be found under 'Not attached to any Care Plan' at the bottom of the page.
