How to sign a care plan using Informed Consent ✍️
Signing a care plan now means creating, linking, and signing a consent form.
Step 1: Create a consent form 📝
Start by creating a consent form for the care / support plan.
Navigate to a service users profile
Go to the ‘Consent Forms’ tab
Click to 'add consent' to create a new form.
When creating the form, you’ll also set a Consent Status. This helps you easily understand where consent currently stands.
Common statuses include:
No status- no progress or outcome has been recorded yet.
Requested – Consent has been requested but not yet completed
Scheduled – A time has been arranged to complete consent
Planned – Consent is intended but not yet arranged
Completed – Consent has been signed and confirmed
Consent not obtained – Consent was not granted
Postponed – Consent has been delayed
Cancelled Consent has been cancelled
Withdrawn – Consent has been removed
You can then select a consent form from the Consent Management Templates Library on the left hand side or create your own using 'Custom entry'
Once your consent form has been selected and status set, ensure you select 'confirm' to save it to the service users profile
Step 2: Link the consent form to the care plan 🔗
Go to the Service user tab
Select a person from your list
Click on Care/Support Plans
Select the relevant care plan
Navigate to the Consent section
Click + Link Consent and select the appropriate consent form
Step 3: Sign the consent form ✍️
Go to the ‘Consent Forms’ tab
Find the relevant care / support plan consent form
Open and review it
Scroll to the bottom and click ‘Sign’
How signing works ⚙️
When signing, the app will guide you through a few questions to confirm the person’s capacity and ability to consent.
If the individual has capacity and can sign 👍
They will:
Sign directly on the form
Confirm they understand the support plan and have been fully informed
If the individual doesn’t have capacity to consent 👥
You’ll be asked to select a Legal Power of Attorney or nominated person (e.g. family member or close friend).
They will:
Sign on behalf of the individual
Confirm they understand the support plan
Note: They must be physically present to sign.
If the individual isn’t able to sign ✋
The same process applies:
Select a Legal Power of Attorney or nominated person
They sign on the individual’s behalf
They confirm understanding of the support plan
Note: They must be physically present to sign.
