Add and have clients sign consent forms in the care office

Accessing consent forms

1. Sign into the Care Office

2. Head over to the ‘Clients’ tab

3. Select which client you would like to create a consent form for

4. Click on ‘Consent’ in the sub menu

Step 1 – Click ‘Add consent’

To add a new consent or lack of capacity form click 'add consent' in the top right corner and choose, if you want to add a consent or capacity form. From this page you can also view which consent and lack of capacity forms have already been signed as well as when they were signed last.

Step 2 – Select which consent / lack of capacity form(s) you want to add

Choose which consent / lack of capacity forms you would like to add from the left-hand menu. You can add one or multiple forms at the same time and even create a custom entry by clicking on the ' custom entry' button.

Step 3 (optional) - Edit consent / lack of capacity forms selected

Click on the pen icon in the top right corner of the form you want to edit. You can then customise the text in the consent / lack of capacity form. Click 'Save' to confirm your changes.

Step 4 - Choose time for signature and review

You can choose to add a signature now or later and also add a review date for your chosen form(s).

Step 5 – Add a signature for the consent / lack of capacity form(s)

Once you’ve clicked on ‘Finish form’ you’ll be asked a series of questions to ascertain the capacity and ability of the individual in your care. If the individual has capacity to consent and the ability to sign, the individual will be asked to sign and check a box to confirm that they have been informed about the decision and consent to it.

What happens if somebody lacks capacity to provide consent?

You’ll be prompted to select a Legal Power of Attorney or a nominated person for the client. This is typically a partner, family member or close friend.

They’ll be asked to sign on behalf of the individual in your care and check a box to confirm that they’ve been fully informed of, and understand what they are providing consent for.

Note: this person will have physically be present to sign.

What happens if somebody doesn’t have the ability to sign?

Just like capacity, you’ll be asked to select a Legal Power of Attorney or a nominated person for the client. This is typically a partner, family member or close friend.

They’ll be asked to sign on behalf of the individual in your care and check a box to confirm that they’ve been fully informed of, and understand what they are providing consent for.

Note: this person will have physically be present to sign.
