Future Plans: Advanced Statements, Consent Forms and Risk Assessments

Contingency Plans have been renamed Future Plans, and you can now add an Advanced Statement alongside your Contingency Plans. You can also link Consent Forms and Risk Assessments directly to these plans, so everything relevant is in one place.

What's changed?

  • Contingency Plans are now found under Future Plans — everything about how you create and manage them works exactly as before.

  • You can now also add an Advanced Statement to a Future Plan.

  • Consent Forms can be linked to Future Plans, the same way they're linked to Care Plans.

  • Risk Assessments can be linked to Contingency Plans, the same way they're linked to Care Plans.

Where to find Future Plans

Go to:

Service user → Profile → Care Plans → Future Plans

This page was previously called Contingency Plans. If you've used it before, everything works the same way.

Adding a Future Plan

  1. On the Future Plans page, select + Add new plan.

  2. This creates a new plan draft, which you can then add scenarios to.

  3. Select Add new scenario and choose either:

    • Contingency Plan, or

    • Advanced Statement

Contingency Plans

Adding a Contingency Plan works exactly as it did before, no changes to the fields or steps involved.

Advanced Statements

To add an Advanced Statement:

  1. Select + Add Advanced Statement.

  2. Enter a title as this is required before you can save.

  3. Optionally, add a description with any further details.

  4. Save the entry.

You can delete an Advanced Statement at any time, whether you're still working on it or reviewing it later.

A few rules to keep in mind:

  • A Future Plan can only contain one Advanced Statement.

  • You can add multiple Contingency Plans alongside it.

  • If a plan already has Contingency Plans, you can still add one Advanced Statement.

Linking Consent Forms to Future Plans

You can link a Consent Form to a Future Plan in the same way you already link Consent Forms to Care Plans.

How it works:

  • Link one or more Consent Forms to a Contingency Plan multiple Consent Forms can be linked to a single plan.

  • From a Consent Form, you'll see a new Linked Future Plans section, which appears below Linked Care Plans and above the Consent Description.

  • You can also link Future Plans directly from the Consent Form while editing or reviewing it.

  • If you manage consent in bulk, you can link Future Plans as part of that process too.

Who can do this:

  • Senior Carers, Senior Carers+, and Admins can link Future Plans to Consent Forms in the Care Office.

  • Carers can also link them from the app.

Linking Risk Assessments to Contingency Plans

You can link a Risk Assessment to a Contingency Plan in the same way you already link Risk Assessments to Care Plans.

How it works:

  • Link one or more Risk Assessments to a Contingency Plan, multiple Risk Assessments can be linked to a single plan.

  • You can link Risk Assessments both when you create a new Contingency Plan and when you review an existing one.

  • From a Risk Assessment, you'll see any linked Contingency Plans listed, the same way linked Care Plans appear.

  • Any linked actions on those Risk Assessments are visible in the same way they are for Care Plans.

  • On the Contingency Plan list, you'll see how many Risk Assessments are linked to each plan select this to jump straight to them.

Who can do this:

  • Senior Carers, Senior Carers+, and Admins can link Risk Assessments to Contingency Plans in the Care Office.

  • Carers can also link them from the app.

In Summary

  • Contingency Plans now live under Future Plans nothing about how you use them has changed.

  • You can now add an Advanced Statement to a Future Plan, alongside any Contingency Plans.

  • A plan can have only one Advanced Statement, but as many Contingency Plans as needed.

  • Consent Forms can be linked to Future Plans, and Risk Assessments can be linked to Contingency Plans — both work the same way linking already does for Care Plans.

  • Senior Carers, Senior Carers+, and Admins can manage these links in the Care Office; Carers can link them from the app.
