The London Care Record (LCR) is a secure digital system that brings together health and social care information from different organisations across London and nearby regions.
It gives health and care professionals a single, comprehensive view of a person’s medical and care history - helping them make faster, safer, and more informed decisions.
💡 Why the London Care Record Matters
The LCR is designed to improve the speed, safety, and quality of care by making sure the right people can access the right information at the right time.
With the LCR, care professionals can:
Make quicker and better-informed decisions about treatment and care plans
Reduce the need for patients to repeat their medical history to multiple professionals
Improve communication and coordination between GPs, hospitals, community services, and social care providers
🌍 Coverage Across London
South East London (SEL) is fully connected to the London Care Record across all its health and care providers.
Other London regions are still in different stages of rollout and integration.
SEL’s system is more advanced because it evolved from the earlier “Local Care Record” (Connect Care) — a foundation that newer regions are still building towards.
🔐 Who Can Access the LCR?
When preparing to deploy the London Care Record, care homes will be sent a training pack by the OneLondon and/or NHS England team, and a Deployment Checklist by Log My Care. These materials highlight the additional steps the care home needs to take when deploying the LCR. The training pack also outlines the clinical risks that the care home need to review and accept, as well as the steps required to deploy the LCR.
Anyone working within an Integrated Care Board (ICB) in London can access the London Care Record free of charge.
To view a record, you’ll need:
A valid Date of Birth (DOB)
A valid NHS Number

Please note, the LCR integration is currently available only for residential care homes.
🧭 Where to Find It in Log my Care

Once we have received the relevant details from the One London team and the London Care Record is enabled for your care service:
You’ll see the London Shared Care Record directly within a service user’s profile.
You’ll also be able to view their Universal Care Plan (add link here).
🔓 Access Levels & Permissions
Access to the London Care Record is controlled through Attribute-Based Access Control (ABAC). Permissions are independent from your role within Log my Care.
The access level can be selected in the Roles & Permissions section of a team member's profile titled “London Care Record Access Level”.
Here’s what each access level allows you to see:
Access Level | Description |
No Access | No access to the London Care Record |
Admin Access | Can view non-clinical information such as alerts, appointments, and visits/encounters |
Senior Carer Access | Can view: discharge summaries, alerts, allergies, appointments, social care details, diagnoses, family history, issued medications, lab results, letters, maternity info, medications (current and past), care plans, procedures, progress notes, referrals, social history, visits/encounters, and vital signs |
Registered Manager Access | Has all Senior Carer access plus: GP records, additional GP reports, clinical document reports, community information, clinical correspondence, endoscopy, and mental health data |

Every access to the London Care Record is logged automatically for transparency and data protection.
You can view who has accessed a record by checking the Audit Trail.

Please note, if there isn’t enough information to match a record, the care record will not load properly. This means it won’t appear in the left-hand menu for that service user - but you’ll still see it for any users who do have a valid record available.
