GP Connect is a service that enables authorised care professionals to quickly and efficiently share and view GP records for those in their care.

To view NHS records for your clients, there is a list of pre-requisites:

  • Need to be CQC-England Registered service

  • Completed the annual DSPT

  • Client consent and NHS number

  • Signed Data Sharing Agreement

You can then access your client's read-only GP records from their profile.

Setting Up GP Connect

  1. Sign the National Data Sharing Agreement, selecting Log my Care as your GP Connect supplier.

  2. Obtain your ODS code, required for signing the agreement.

  3. Notify us once the agreement is signed, providing your ODS code.

  4. Wait up to 2 weeks for GP Connect to provide your integration code (ASID).

  5. Setup complete! You'll be notified when your integration is activated.

Please note that you'll also need your ODS code to sign the agreement.

GP Connect Onboarding Checklist

  • Ensure all relevant service users have their NHS number entered in their profile (Care Related Information section).

  • Sign the GP Connect Privacy Notice for each service user before accessing records:

    1. Click on Consent.

    2. Add a new consent form.
