How to Archive a Risk Assessment?

Move old risk assessments to the archive list. This keeps active lists easy to use, while still keeping a full history for compliance.

Archiving helps you focus on current assessments without losing important past records.

Archiving a risk assessment is simple! Follow the steps below:

Go to the Risk Assessments page

  1. Select the assessment you want to archive and click Edit.

  2. Scroll to the bottom and select Archive Risk Assessment. Enter the reason for archiving and click Confirm.

Once confirmed, the assessment is removed from active use, detached from any connected care plans, and its list item in the template resets.

Q: Where can I find archived risk assessments?
โ€‹A: Archived assessments appear in their own section:

  • Scroll to Archived Risk Assessments.

  • Click on โ–ผ to collapse the section.

  • For a specific service user, archived assessments appear under Archived Risk Assessments.

Q: How can I view an archived risk assessment?
A: When viewing an archived assessment:

  • A banner shows the archive date, who archived it, and the reason.

  • The Restore Risk Assessment option is also available.

Q: How do I restore an archived risk assessment?
โ€‹A: To restore an assessment:

  • Click Restore Risk Assessment.

  • The assessment will move back and will not be attached to any Care Plan.
