How do I add goals?

Step 1: Click on the client you’d like to add a goal for 🧍


In your client’s profile under the ‘Care Planning’ section, on the left-hand, click on ‘Goals’ and locate the blue ‘Add goal’ button on the right.

Step 2: Add the goal title, description and measure of success 📝


Start off by giving the goal a title and completing a description for further details outlining what your client wants to accomplish, why they want to accomplish this and what strengths they already possess that they can use to achieve this goal.

In the ‘Measure of success’ box, you’re able to explain how you’ll know that the goal is achieved for your client in order to assess the status of the goal.

Step 3: Set a duration for this goal to be achieved within 📆


Here you will have the opportunity to add the date you’d like this goal to start and when it should be achieved by.



❗️Importance


Adding an importance level will allow for you to track how important a goal is to a client. Level 1 indicates the lowest level of importance whilst Level 10 indicates the highest level of importance.

💪 Action Plan


The action plan will outline the steps the client will be planning to take to achieve this goal. On the right-hand side of the goals, you will see a drop-down menu with a status depending on the progress a client has made with the goal. If you’d like to delete a goal, simply click on the trash can icon on the far right.

🔗 Linking a Support Plan


You can link a Support Plan to a goal by clicking ‘Link Support Plan’ and selecting a Support Plan that has already been created.

🗂️ Documents


You may choose to attach a document to this goal for a number of reasons and you can choose a category once you’ve clicked on the ‘Add Document’ button.

Step 4: Add any additional notes and set a ‘Next review’ date 🗒️


The notes box will allow you to add any further information regarding the goal and set a ‘Next review’ date. The date will need to be within the duration in which the client aims for the goal to be achieved.

Step 5: Set the status of the goal 📈


Lastly, you will need to set the status of the goal. This will give an overview of the progress and allow you as a care home manager to track the overall progress your client has made.
