Please note: Resident transfers can only be completed for customers on our Outstanding Plan who have multiple sites within the same Home Group.
We require 1–2 weeks' notice before the desired transfer date to allow sufficient time for the transfer to be scheduled and completed correctly. Unfortunately, we are unable to accommodate same-day resident transfer requests.
To transfer a resident from one site to another, an admin user must log in to the Group Site and click the Transfer Resident button shown below.
Clicking this button will start a chat requested the necessary information for our Support Team to complete your request. Please have the following information ready to provide our team:
Service User ID (required) - this can be found via the browser URL (please see the screenshot for reference).
Service User Full Name (optional, but recommended)
Current Site Name (required)
Destination Site Name (required)
Preferred Transfer Date – the date the transfer should ideally be completed by
No Later Than Date (required) – the latest acceptable date the transfer must be completed by
Once this information has been received, your request will be passed on to our Support Team, who will complete the transfer for you.

Resident profile information
Logs
Care Plans
Risk Assessments
Initial Assessments
Incident Reports
Goals
Documents
Body Maps
eMAR Medications
eMAR Rounds
The following data will not transfer and will be permanently lost:
Tasks
Recurring Tasks
Family Access accounts
Audit Trails
