It scores care plans across six areas and highlights specific findings that may need attention. Each finding includes a link to the record it relates to, so you can review and update it directly.
You can find Care Plan Audits in the Insights Hub, under the Quality tab.
⚠️ AI-generated and Early Access
Care Plan Audits looks at the quality of your documentation, not the quality of care being provided.
It is not a regulatory or clinical assessment, and it cannot predict a CQC rating. The scores are intended as guidance to help you identify areas where your care plan documentation may need improvement.
Running an audit for the first time
By default, the audit will include every home you have access to. After it has been run the first time, you can choose specific homes if you only want to audit certain ones:
Running subsequent audits
⚠️ Care Plan Audits can be run once for free, subsequent re-runs require a paid subscription
By default, the audit will include every home you have access to. You can choose specific homes if you only want to audit certain ones.
Open the Home filter at the top of the page.
Select the home or homes you want to include, or leave the filter empty to include all your homes.
Select Run audit/Re-run audit
The audit will run in the background. You’ll see a progress indicator showing how many people have been audited.
You don’t need to keep the page open while the audit is running.
A full audit can take a few minutes, depending on how many people and homes are included.
Understanding your dashboard
Once the audit has finished, your dashboard will show:
Your Overall Quality Score
When the audit was last run
How many people and homes were included
You can hover over the ⓘ icon next to a category to see a short explanation of what it checks and which CQC regulation it relates to.
Understanding the scores
Each category is scored as a percentage:
Score | Band |
0–39% | Low |
40–59% | Average |
60–79% | Good |
80–100% | Outstanding |
People without an active care plan are automatically excluded from the scores.
If anyone is missing an active care plan, you’ll see a banner at the top of the dashboard. Select Review to see who is affected and open their care plans.
The six quality categories
Each care plan is scored across six categories. These scores look at what is written in the documentation rather than making a clinical judgement.
Depth
Checks whether the relevant care-planning sections are completed with meaningful, person-centred information. (Regulation 9 and Regulation 17)
Risk Alignment
Checks whether recorded risks have clear and specific guidance explaining how staff should support the person. (Regulation 12)
Consistency
Checks whether information is consistent across care plans, risk assessments and the person’s profile. Flags contradictions, not absences. (Regulation 17)
Grammar
Checks whether the documentation is clear and easy for staff to understand and act on safely. Clarity that affects interpretation, not cosmetic style. (Regulation 17)
Specificity
Checks whether the care plan gives clear, practical instructions rather than vague statements. May note guidance but never supplies the missing clinical detail itself.(Regulation 9 and Regulation 12)
Independence Promotion
Checks whether the care plan captures the person’s strengths, abilities and choices, including how staff should support their independence. (Regulation 9 and Regulation 10)
Reviewing a category
You can select any category in the Score Breakdown by Category card to see more detail.
Select a category, such as Depth.
A panel will open showing:
The score for that category across your homes
The five lowest-scoring people
The main issues contributing to the score
Each finding explains what may be missing or unclear and links to the relevant record.
Select View related sources under a finding to see the care plans or risk assessments the finding is based on.
You can then select a linked record to open it.
Taking action on a finding
Each finding links directly to the record it relates to, so you can review and update the documentation without having to search for it separately.
Select the linked record under a finding.
If the record belongs to a different home, you’ll be asked to confirm that you want to switch homes before it opens.
Review and update the record in Care Office as needed.
Return to the Care Plan Audits tab.
Select Actioned on the finding to confirm that you’ve reviewed it.
⚠️ Actioned cannot be undone
Marking a finding as Actioned creates a permanent record that you have reviewed and edited the related documentation.
Once confirmed, the finding will be marked as Actioned and moved to the bottom of the list. This makes it easier to see which findings still need attention.
Dismissing a finding
If a finding isn't relevant to the person or you don't agree with it, you can dismiss it instead.
Select Dismiss on the finding.
Enter a short reason for dismissing it.
Confirm the dismissal.
A reason is required before a finding can be dismissed. Your reason helps us improve future audit findings.
Dismissed findings are marked as Dismissed and moved to the bottom of the list.
Please note that a dismissed finding may appear again when you run a new audit if the underlying issue has not been addressed.
Reviewing a home
You can also select a home from the Scores by Home card.
This shows:
The scores for each category
The lowest-scoring people in that home
The main issues identified
You can also Action or Dismiss findings from this view.
Reviewing scores by person
The Scores by Person We Support table shows everyone across your selected homes, along with:
Their overall score
Their score for each of the six categories
Select a column heading to sort the table by that score.
Select a person’s name to open their individual score breakdown and findings.
Sorting by a category can help you quickly find documentation that you want to review. For example, you can sort by Grammar to find lower-scoring documentation that may need attention.
The individual view shows the person’s scores across all six categories and their specific findings.
You can Action or Dismiss findings from this view in the same way as from the category and home views.
