How can I change the outcome on the MAR chart?

To ensure your MAR Chart accurately reflects what happened during medication administration, you may sometimes need to update an outcome.

For example, if a medication was initially recorded as Refused but was later accepted and administered, the MAR Chart can be updated in the Care Office to reflect the correct outcome.

Please note: Only administrators with medication permissions can update MAR Chart outcomes in the Care Office.

There are two ways to update an outcome on the MAR Chart:

  • Changing the outcome for a missed signature

  • Changing the outcome for a medication that has already been signed


Changing the outcome for a missed signature

Step 1: Select the missed record

Locate the missed medication record on the MAR Chart and click on it.

Select Sign for missed medication.

Note: This option is only available for medications that do not require a second signature.


Step 2: Complete the administration details

You will be presented with a form similar to the one used in the Carer App.

Complete the relevant administration details, including:

  • The outcome

  • Who administered the medication

  • Any required notes or additional information


Step 3: Save the record

Once you've completed the form, click Save.

The missed record on the MAR Chart will be updated and the outcome code will change to reflect the new administration outcome.



Changing the outcome on the MAR chart for a signed medication

Step 1: Select the administration record

Locate the administration record on the MAR Chart that you would like to update and click on it.


Step 2: Update the outcome

Under the Outcome field, select the correct administration outcome from the dropdown menu.


Step 3: Save your changes

Once you have selected the correct outcome, click Save.

The updated outcome will now be reflected on the MAR Chart, and the administration code will update accordingly.

For audit purposes, both the original record and the updated record will be visible in the Log Revisions section.
