Incident logs marked as safeguarding issues are used to record serious incidents and concerns. They are handled differently from other incidents to make sure sensitive information is only visible to the right people.
Once you submit a safeguarding log, only Safeguarding Officers at your site will be notified and able to view it. You won’t see the log again after it’s submitted.
🛡️ Steps to raise a safeguarding issue
Open the Care App.
Choose a resident/service user.
You can either:
Go to Daily log and select the resident/service user.
Or, open the resident’s profile, go to Logs, then tap + New log.
Select the category: Incident.
Choose the type of incident: Safeguarding.

Enter details about the safeguarding concern:
Who is the concern about?
What is the safeguarding concern?
Choose from: Physical abuse, Emotional abuse, Neglect, Sexual abuse, Financial abuse, Discriminatory abuse, Organisational abuse, Domestic violence, Modern slavery, Self-neglect, or Other.
Describe what happened in your own words.
What date did this take place?
What time did this take place?
Is the person safe right now?
What actions have been taken so far?

7. Add supporting information (optional):
Add up to 3 photos/documents and 1 video.
Add a witness to the log.
Add a location.
⚠️ Please note, safeguarding logs can’t be flagged for handover or linked to goals.
8. Submit the log.
You’ll see a confirmation message that your safeguarding log has been submitted successfully.
After this, you won’t be able to view the log.
🧠 Important notes
Only Safeguarding Officers are notified of safeguarding logs and can view them in the Care Office.
This restriction ensures that sensitive information stays confidential and is handled by the right people.
You can also submit a Safeguarding Log using Care Office, but you won’t be prompted to add as much detail and will need to include it in the description.
